How CLA Works

Those eligible for the program receive a CLA Membership Card at a cost of $49.95 for one full year of chiropractic treatment.  The savings apply for the cardholder and the immediate family members covered on their tax form.  CLA cannot be used in conjunction with any third party payer such as health insurance, personal injury claims, workers compensation, or government insurance programs.

There are no claim forms to fill out, exclusions, waiting periods, deductibles, or visit limits. Simply show your Chiropractic Lifecare of America membership card to Dr. Sheila at each visit to enjoy the benefits of unlimited affordable chiropractic care.

Sign Up Three Ways

1.  In our office during your first visit.  When you call us at 770-760-1394 to schedule your first appointment, you can fill out the membership form and pay the membership fee to us and we will mail it directly to CLA for you.  (Recommended)

2. Download the membership form, and bring the completed form to your first appointment.  We will mail it directly to CLA for you.

3.  Visit the official CLA website and fill out the online form.  If you choose this method, you will need to provide proof of registration on your first visit as it takes several weeks to receive your CLA Membership Card (the official proof of membership).  You may need to provide our Registered Agent (RA) Number 1566 in order to apply.
